More information:http://www.cic.gc.ca/english/information/faq/visit/index.asp#superVisa

I would recommend you to wait for the specific requirements on minimum income and private medical insurance. There’s no need to rush; it’s next Thursday anyway. GL[s:277]

Well, what they put on the CIC official website is “prove that they have bought Canadian medical insurance for [backcolor=#ffff00]at least one year[/backcolor] to cover the period of time that they will be in Canada”. It seems regardless how long your parents would be here, one year Canadian medical insurance is mandatory. It’s a smart move of CIC for the benefit of Canada, i.g. boosting insurance business, job creation, reduce government burden, etc.. So I don’t believe they will change/remove it.

I know someone who paid around $20,000 on medical treatment for her mom who was in an accident during her visit and had no medical insurance at that time. Basically you pay everything from your own pocket if there’s no insurance coverage. Of course we hope nothing bad happens, but we still need to prepare for the “what if”.

The regular visitor visa has no requirement of medical insurance and is valid for 6months.  [s:277]

If you are asking whether you have to extend the medical insurance after one year, I believe the answer is “no”.  I called CIC Friday because I didn’t know the answer either. The agent told me that because there’s no specific requirement on extending the insurance, it is up to the applicant to make the decision. It’s recommended to extend the medical insurance if your parents are staying for more than a year, but not mandatory. Regarding the one year medical insurance:
[li]  covers the applicant for health care, hospitalization and repatriation; [/li][li]  provides a minimum of $100,000 coverage; and [/li][li]  is valid for each entry to Canada and available for review by the examining officer upon request (CIC Operational Bulletin 357)[/li]
I believe oversea’s comment on insurance premium ($4000/person; $8000/couple) is a bit high. I used Manulife’s quote tool to estimate my parents’ premium cost. It’s about $3500 (in total) a year. Please keep in mind that parents’ age and health condition have direct impact on the premium; the older your parents are, the higher premium you may have to pay. Hope it helps.
